PRISM – New Web Portal Keeps
Referring Physicians in the Loop
Giving referring physicians improved access to patient data
is at the crux of Stanford Hospital & Clinics (SHC) implementation
of a new web-based physician portal for Epic – SHC’s electronic medical records system.
In September,Physician Referral Information at Stanford Medicine (PRISM) was launched. Replacing previous portal systems, PRISM gives referring physicians a more robust link to the patient
data contained within Epic, allowing them to communicate
with Stanford more easily, review lab and imaging data, and
obtain records for the care their patients receive at SHC.
According to Stanford Chief Medical Information Officer
Pravene Nath, MD, the newly enhanced PRISM web portal
is, “the fastest and most state-of-the-art communication
between SHC and its referring physicians.” More than 500
physicians are currently using PRISM.
Once enrolled, referring physicians will find that most information
from their patients’ health records is automatically
available in real time. This includes:
- Clinic visit notes
- Lab results
- ED summaries
- Radiology reports/images
- Progress notes
- Discharge summaries
- Procedure notes
- Other data relevant to continuation of care
Referring physicians can also designate members of their
office staff to access the system, allowing them to download
and print records.
PRISM does not replace the traditional physician-physician
follow-up communication that occurs via letter, fax and
phone. But, it does give referring physicians more timely access
to the information they need.
